Chesser Surgery is seeking a motivated Practice Nurse to join our team and provide high-quality care to patients.

This is apart-time role (16 hours per week) but we are open to discussing flexible arrangements to suit the right candidate.

Main duties of the job

Main duties of the job

As a Practice Nurse, you will:

Conduct comprehensive assessments of patients health needs.

Develop, implement, and review individualised care plans.

Provide appropriate treatments and interventions as required.

Undertake treatment room responsibilities and deliver Chronic Disease Management clinics.

Carry out specific clinical tasks, including but not limited to :

Travel health consultations and vaccinations

Cervical screening tests

Childhood immunisations

Blood pressure monitoring and ECGs

Venepuncture and health checks

Work collaboratively with GPs, other nurses, and the wider healthcare team to ensure high-quality patient care.

Maintain accurate, timely, and confidential patient records in line with NHS standards.

Deliver health education, advice, and counselling to patients and their families.

Keep up to date with current clinical guidelines, medical advancements, and best practice in primary care.

Chesser Surgery is a well-established and trusted provider of NHS services in Carshalton, Surrey. We have 6200 patients, 1 Senior Partner, 1 3/4 time Junior Partner and 2 salaried GP's as well our nursing team and non-clinical staff members.

Job responsibilities

Chesser Surgery is seeking a motivated and experienced Practice Nurse to join our team and deliver high-quality patient care.

This is a part-time role (16 hours per week, Monday-Friday). We are ideally looking for a qualified General Practice Nurse,

The role involves a wide range of clinical responsibilities, including patient assessments, care planning, treatment room duties, and the delivery of Chronic Disease Management (CDM) clinics. You will also undertake specific tasks such as travel health consultations, cervical smear tests, childhood immunisations, wound care, venepuncture, and routine health checks.

Working closely with GPs, fellow nurses, and the wider healthcare team, you will play a key role in ensuring patients receive safe, effective, and compassionate care. You will maintain accurate patient records, provide health education and counselling, and keep up to date with current clinical guidelines and best practice.

Person Specification Qualifications
  • Registered Nurse (NMC registration)
  • Evidence of current NMC revalidation
  • Experience in primary care or transferable clinical setting
  • Competence in immunisations and vaccinations (adult and childhood)
  • Cervical cytology training
  • Wound care and dressings experience
  • Ability to manage Chronic Disease Management (CDM) clinics (e.g., diabetes, asthma, COPD, hypertension)
  • Good IT skills and ability to use electronic patient records
  • Previous experience working as a Practice Nurse in General Practice
  • Travel health training and vaccination experience
  • Additional qualifications in chronic disease management (e.g., diabetes diploma, respiratory training)
  • Evidence of ongoing professional development (CPD courses, certificates)
  • Mentor/students supervision & assessment (SSSA) qualification for supporting students or junior staff
