We are seeking an experienced and dedicated PracticeNurse to join our GP practiceteam. As a Practice Nurse, you will be responsible for delivering a range ofhigh- quality clinical services and supporting the practice in the provision ofexcellent patient care.
Working as amember of the multidisciplinary practice team, the post holder will ensurenursing services are delivered effectively to the entitledpatient population.

Main duties of the jobThe PracticeNurse will provide nursing services including adult and child immunisations,wound care, cervical screening, long-term condition management, medicationadministration and contraception checks. They will be involved in severalclinical areas such as infection prevention and control, cold chain management,health promotion, and supporting the practice management team in reviewing anddelivery of clinical policy and procedure.

Job responsibilitiesThe followingare the core responsibilities of the Practice Nurse. There may be, on occasion,a requirement to carry out other tasks.This will be dependentupon factors such as workload andstaffing levels:
- Conducting health assessments, screening, and triage services,and providing appropriate follow-up care.
- Undertaking a range of clinical procedures, including wound care, immunisations, cervical screening,and phlebotomy.
- Assessing and supporting patientswith chronic conditions, such as diabetes,asthma, and COPD.
- Administration of medication inaccordance with national patient group directions or patient specificdirections.
- Implementing vaccinationprogrammes for adults and children
- Reviewing patient medicationsto enhance compliance
- Providing healtheducation and promotingpreventative care to patients.
- Maintaining accurateand up-to-date patientrecords and documentation.
- Supporting the clinical team with administrative tasks and participating in the developmentand implementation of clinical protocols and guideline.
- Ensuring that all clinicalactivities are conductedin line with relevantregulations, standards, and guidelines.
- Maintaining accurate clinicalrecords in conjunction with extant legislation.
Immunisations:
- Weekly ordering of vaccines
- Vaccine stock take and rotationof stock
- Maintenanceof Cold Chain and reporting any breaches
- Ensure data loggers and fridge temperature records are uploaded to TeamNet
- Review of QOF targetsand working with care coordinator for recalls
- Arrange seasonal immunizationclinics
- Maintain the medicinesmanagement and cold chain policies
Infection Prevention and Control:
- Undertake routine IPC audits inaccordance with local policy
- Promote best practice ininfection prevention control, supporting other members of staff to uphold localIPC policy.
- Regular infection control updatesfor staff
Diabetes:
- Undertakeannual diabetes review including results interpretation, patient education andmedication optimisation
- Participatein the diabetes working group
- Organise group consultations for patients
- Organise health promotion sessionsfor diabetes
- Organise diabetes consultant sessionsand DSN sessions
Asthma:
- Undertakeannual asthma reviews including reviewing diagnoses, asthma control, medicationuse, inhaler technique, and regime changes to improve symptom control.
- Participate in the asthma workinggroup
- Plan and support implementation of robust call/recall system with care coordinators

- Minimum of one years experience working as a practice or community nurse

- Experience in using clinical systems and electronic patient records including Docman and EMIS
- Proven ability to evaluate the safety and effectiveness of their own clinical practice
- Interpreting and implementing local and national policy agendas for health
- Experience of working in a primary care environment
- Experience in management of long-term conditions e.g. asthma, COPD, diabetes, CHD
- Current registration with the Nursing and Midwifery Council (NMC)
- Post graduate diploma or degree (Chronic Disease Management)
- Wound care/removal of sutures and staples
- Immunisations (routine, seasonal, childhood or travel)
- Understanding the importance of evidence-based practice
- Understand the requirement for PGDs and associated policy
- Ability to record accurate clinical notes
- Ability to work within own scope of practice and understanding when to refer to GPs
- Chaperone procedure
- Knowledge of clinical governance
- Compression bandaging and ABPI
- NHS Health Checks
- Chronic disease management
