Practice Nurse - Reading

Location: Reading, UK.
Schedule: Monday and Wednesday 08 00. Start ASAP.

Responsibilities

• Implement evidence based processes for managing patients with long term conditions, including care for elderly and housebound patients.
• Deliver nursing services as a senior member of the multidisciplinary team.
• Provide care in infection prevention and control, health promotion, chronic disease management, well woman and well man clinics.
• Support the practice management team in reviewing and delivering clinical policy and procedure.

Essential Qualifications

• Registered Nurse with the Nursing and Midwifery Council.
• Experience in chronic disease management (diabetes, hypertension, respiratory conditions).
• Previous practice nurse experience.
• Qualified smear taker.
• Immunisations/Vaccinations trained, including childhood immunisations.
• Clear DBS, right to work, etc.

Desirable

• Experience using EMIS.
• Diabetes qualification.
• Asthma/COPD qualification experience.
• Travel health knowledge.
• V300 prescriber.
