Practice Nurse - Locum - Flexible hours - Slough

Dream Medical are looking for an excellent Practice Nurse to specialise in Chronic Disease management within a friendly GP surgery in Slough. You will join an efficient team of GPs, ANPs and HCAs who are committed to delivering exceptional patient care. Days and times are flexible for the right candidate.

Responsibilities

• Conduct asthma reviews and COPD reviews
• Manage chronic disease patients, including diabetes and other chronic conditions
• Perform spirometry and other routine assessments
• Administer travel vaccinations, B12 and other injections, and elderly checks
• Carry out routine cytology reviews and contribute to quality improvement initiatives
• Provide care in line with QOF and NHS guidelines

Qualifications & Requirements

• Relevant nursing qualifications and experience in primary care
• Enhanced DBS issued within the last year
• Registered with the NMC and holding a valid PIN
• Up to date with all required certificates (as listed above)
• Provide two clinical referees
