Practice Educator - Community Falls

The Specialist Falls Service within Hampshire and Isle of Wight Healthcare has an exciting fixed-term opportunity for an enthusiastic and motivated healthcare professional who is passionate about falls prevention, education and improving patient outcomes. This role will play a pivotal part in delivering our Community Falls Transformation Programme, helping to develop a more consistent, equitable and sustainable approach to falls assessment and management across Hampshire and the Isle of Wight. Initially funded until March 2027, with the potential for extension subject to further funding, the post offers a unique opportunity to influence practice at scale, support workforce development and contribute to reducing avoidable harm for some of our most vulnerable patients.

As Falls Practice Educator, you will work closely with community nursing, therapy, urgent care and specialist teams to deliver quality education, develop competency and confidence in falls assessment and prevention, and champion evidence-based practice. You will support staff through training, clinical supervision and quality improvement initiatives, helping to embed a culture of continuous learning and excellence across community services. This is an ideal role for a clinician with a passion for education who wants to shape the future of falls prevention, improve equity of access and leave a lasting legacy across our newly formed Trust.

As this is a trust wide role, it is essential that the successful candidate is a car driver.
