Are you a talented and patient-focused clinician looking to make a difference? Our client is seeking a Specialist Physiotherapist to join their passionate team, based in Southampton. This community-based role offers the opportunity to deliver personalised care in patients' homes, facilitating early supported transfers from hospital or preventing unnecessary admissions.

Responsibilities

• Conduct comprehensive holistic assessments of patients.
• Formulate individualised physiotherapy treatment plans.
• Deliver physiotherapy sessions in home or community settings to maximise patient independence.
• Educate patients and carers on the correct use of mobility and functional equipment.
• Travel regionally to visit patients at home, requiring a full UK driving licence.

Qualifications

• Previous community experience or a track record of working autonomously.
• Experience in an acute care setting.
• HCPC registration.
• Strong IT skills.
• Willingness to cover three different shift patterns, 5 days out of 7.
• A full UK driving licence, comfortable with driving in and around Southampton.
