Job responsibilities Key Duties / Responsibilities
1.
Working in a person centred way to promote and maximise independence, understanding the individuals needs, their aspirations and goals.
2.
Contribute to and inform individual care act assessments and individualised care plans to enable individuals and reduce any long term care needs.
3.
Encourage or assist adults where appropriate with personal care, such as bathing, showering, toileting, hygiene continence care, oral hygiene, skin integrity, meal prep (nutrition and diet) and administering medication including controlled drugs.
4.
To apply infection control and food hygiene measures within the adults home environment and to use protective equipment.
5.
Enabling and support maybe include activities as supervising, monitoring and coaching to empower adults to be as independent as possible and have control over their own lives. You may also be required to escort or accompany adults to outings, appointments or other events. This also includes tailored exercise programs.

Undertake commercial activities including cleaning tasks to maximise the efficiency of the service
7.
To work in partnership with other relevant professionals and external agencies to provide a personalised service to meet individuals needs.
8.
Maintain high standards of care and support, in a safe environment which promotes equality culture, disability, race, age, sexuality, marital or civil partnership status, transgender status or religious belief and sensitivity for all adults.
9.
Flexibility in ensuring that the city council can be a provider of last resort in emergency situations
10.
Use the councils electronic monitoring record system to maintain a record of work undertaken.
11.
Work with autonomy to ensure Enablement and Support Plans accurately reflect the persons needs and agreed level of support by informing management of any changes, always seeking to optimise the participation of the person accessing the service in the design and planning of the care and support they require, take prompt action to resolve any discrepancies or changes to the persons care and support needs.

Adhere to, the teams aims and objectives whilst operating within relevant processes, policies and procedures, within Stoke on Trent Councils policies and procedures, CQC
13.
To immediately elevate any safeguard concerns in order to promote the welfare of vulnerable adults. To report and elevate in accordance with policy and procedures.
14.
Maintain training and continuing professional development. Complete all relevant e learning training as well as face to face.
15.
Ensure comfort, safety, dignity and inclusion whilst also protecting and maintaining confidentiality and having a positive attitude at all times.
16.
Deal with abuse in challenging situations using appropriate de escalation techniques.
17.
Monitor, read and write reports, record accurately on progress towards adult goals, changes in the adults health and wellbeing, needs or circumstances as required. This could include completing electronically.
18.
To support the office (Management) in completing tasks where appropriate to grade.
19.
To support the wider provider market, including domiciliary agencies and homes such as Marrow House.
20.
Take an active part in adults reviews and staff meetings.
21.
Regularly discuss and gain feedback on experiences of the service to drive improvement as well as gaining feedback from adults who access the service. Provide information and advice to adults and informal carers.
22.
Ability to work alone during bad weather and unsociable hours.
23.
Contribute to and follow risk assessments relevant to the adults and within the home environment to ensure the safe delivery of the service including undertaking dynamic risk assessments.
24.
Demonstrate an understanding and work in line with the Mental Capacity Act.
25.
To work across Stoke on Trent (City Wide) to meet the needs of individuals who live in North and South of the city. You are expected to cover all areas by obtaining a current and valid driving licence or through access to public or other transport in line with the Equality Act.
26.

- You will be required to have a genuine interest and commitment to providing high quality care and support to adults in promoting their privacy, dignity, choice and independence.
- These are part time positions only, working 21 hours per week and therefore the salary will be pro rata as shown above.
- You will be required to work on a rota basis which will include weekends.
- This post includes working evening shifts 4 pm- 10pm; we predominantly require evening shifts.
- The role will require you to be able to travel across the city.

Person Specification Experience
- To understand the scope of the role and identify how this may develop over time
- Be person centred, undertake care and support activities in a way that reflects the preferences and independence of adults.
- Have an understanding of common health conditions and how these affect the delivery of care
- Have a friendly and caring approach and a genuine desire to help others
- Be able to administer medication unsupervised and what to do in a challenging situation
- Be willing and able to undertake care certificate qualification and equivalent level 2 in Health and Social Care.
- Willingness to achieve equivalent level 3 in Health and Social Care.
- Demonstrate a commitment to continuous personal development, relevant skills, knowledge and experience to ensure competence in delivering, and overseeing the delivery of, care and support in accordance with regulatory standards
- Demonstrates excellent interpersonal communication skills to be able to liaise and work effectively with people using services, professionals, stakeholders, members of the public and colleagues
- Have a willingness to take Personal Responsibility, taking ownership for decisions and being accountable for what has been promised
- Demonstrate a high level of organisational skills
