We are expanding our service as part of Nottinghamshire County Council's commitment to increasing our capacity and ensuring individuals receive the right care and support at the right time.
As a Care and Support Broker, you will play a vital role in the adult social care pathway. Working closely with social workers, care providers, health partners, and families, you will source and secure high quality, best value care solutions tailored to people's assessed needs. Your work will directly support timely hospital discharges, promote independence, and help deliver positive, person centred outcomes across the county.

Key Responsibilities- Coordinate and broker care that aligns with a person's assessed needs, ensuring support is timely, appropriate, and focused on promoting independence.
- Work closely with care providers, building positive relationships to negotiate care and support, costs and availability that deliver both quality and value. Maintain accurate, up to date records and ensure all activity complies with organisational policies, statutory requirements and best practise standards.
- A core part of the role is making sure every judgement reflects the needs, wishes and well being of the person, supporting them to live a good life.
- Someone who brings both professional skill and genuine compassion to their work. You'll have experience in adult social care and a proficient understanding of the care market, enabling you to make informed decisions and support the provision of high quality care services.
- Excellent communication skills are essential. You'll be working with a wide range of people, sometimes with complex needs, so the ability to listen, explain and build trust really matters.
- You'll be comfortable managing demanding priorities in a fast paced environment, staying organised while keeping the person at the centre of everything you do. Empathy, emotional intelligence, and a commitment to dignity and respect are key. If you can balance clear thinking with human understanding, you're thrive in this role.
