Job Description

Job Title: Personal Advisor (Leaving Care Services)

Job Purpose

To support young people aged 16-25 who are leaving care, helping them transition into independent adulthood. The role focuses on providing practical, emotional, and statutory support to ensure young people achieve stability in housing, education, employment, health, and financial independence.

Key Responsibilities
  • Manage a caseload of care-experienced young people and maintain accurate case records and statutory documentation
  • Contribute to and regularly review Pathway Plans, ensuring young people are fully involved in planning their support
  • Carry out assessments for young people resuming or accessing education, training, or additional support needs (including post 21 support where applicable)
  • Provide advice, guidance, and practical support to help young people transition to independent living
  • Support access to suitable accommodation and help maintain housing stability
  • Work with young people to develop budgeting and financial management skills and ensure access to appropriate financial support
  • Coordinate services across agencies to deliver holistic, person centred support
  • Monitor and review support packages in line with individual Pathway Plans
  • Work in partnership with professionals and external agencies to safeguard and meet young people's needs
  • Promote the welfare, voice, and participation of young people in all decision making processes
Essential Requirements
  • Experience working with and engaging young people, ideally within a social care or support setting
  • Experience working with multi agency professionals in diverse communities
  • Ability to manage sensitive and confidential information in challenging situations
  • Good understanding of relevant legislation including Children Act 1989/2004 and Leaving Care legislation
  • Knowledge of safeguarding procedures and statutory responsibilities for looked after children
  • Strong planning, assessment, and case management skills
  • Full UK driving licence and access to a vehicle
  • Enhanced DBS check required
