Job Summary

As a Clinical Pharmacist at Montpelier Health Centre, you will play a vital role in supporting the general practice workforce and elevating patient care standards. You will contribute to the ongoing improvement of healthcare services in line with the national Clinical Pharmacist programme.

Key Responsibilities

1. Collaborate with the general practice team to optimize medication management.
2. Conduct medication reviews and provide expert advice to enhance patient outcomes.
3. Support the implementation of clinical guidelines and best practices.
4. Participate in the CPPE Pathway: Primary care pharmacy education pathway : CPPE.

Requirements

• Registered Pharmacist with relevant experience.
• Commitment to ongoing professional development.
• Familiarity with the national Clinical Pharmacist programme.
• Excellent communication and teamwork skills.
