PERM PRACTICE NURSE PART TIME HOURS YEOVIL ££NEG + BENS

Dream Medical is working with a Health Care Centre in the Yeovil area who urgently requires a part time Practice Nurse.

• Work across two sites in the Yeovil area.
• Part of the nursing team responsible for ensuring the practice provides optimum care relevant to the practice population, both in the practice and in patients' homes.
• Support patients to be healthy by monitoring long term conditions, and carrying out health prevention and screening activities.

• Relevant experience in Chronic Disease Management.
• Registered with the NMC, with post registration experience.

Salary is negotiable and dependent on experience. A full range of benefits is on offer, including an NHS pension.
