Job Description

TheGeneral Medical Practitioner (GMP) will provide high-quality, patient-centredcare to individuals and families within the Primary Care Network. The GMP willbe responsible for diagnosing and treating a range of medical conditions,promoting health and well-being, and working collaboratively with otherhealthcare professionals to enhance patient outcomes.

Main duties of the job
  • Conduct comprehensive assessments and examinations of patients.
  • Diagnose and manage acute and chronic medical conditions.
  • Develop and implement individualised care plans in collaboration with patients and the healthcare team.
  • Prescribe medications and therapies as appropriate.
  • Provide preventive care and health education to patients.
  • Participate in multidisciplinary team meetings and case discussions.
  • Maintain accurate and up-to-date patient records in accordance with legal and ethical standards.
  • Engage in continuous professional development and training.
  • Support the PCN in achieving health targets and initiatives.
About us

Nexus MK PCN offices are based in Linford Wood Milton Keynes and cover 5 practices which include Oakridge Park Medical Centre, Wolverton Health Centre, The Stonedean Practice, Sovereign Health Centre, Purbeck Medical Centre.

  • Special interests in specific areas of medicine (e.g., mental health, chronic disease management).
  • Familiarity with electronic health record systems.
  • Knowledge of current NHS policies and guidelines.
