ABOUT THE ROLE

We are looking for anexperienced Advanced Clinical Practitioner, independent prescriber, with adigital badge in advanced clinical practice to in our care home team to provideregular ward rounds in care homes. Providing clinical management of care homeresidents presenting with acute and non-acute medical needs, undifferentiated andlong-term condition presentations. You will be developing a management plan followingrobust clinical consultations including assessment, examination, investigation,diagnosis, care planning, treatment and evaluation of care.

Main duties of the job This will also includeundertaking medication reviews, new resident assessments, post hospitaldischarge reviews, and ReSPECT planning with residents and relatives, to ensurea patient focussed approach to end-of-life care.

You will build good workingrelations with the care home staff make a difference to the quality of healthcareprovided to residents in Rushcliffe PCN care homes.

We are looking for an ACP whowishes to extend their expertise in frailty management and be part of a dynamicteam to support the delivery of the National Enhanced Healthcare in Care Homes,Direct Enhanced Service Framework (DES).

You will support the delivery ofCOVID and flu programme, by clinically screening and vaccinating care home residentsas part of the Rushcliffe and National vaccination programme

The post holder will work acrossorganisational boundaries, fostering good relationships between providers andstatutory bodies and improve care for people living in long term residentialand nursing care.

This job requires the postholderto travel and have face to face consultations with residents in care homes,undertake remote consultations and discussions, liaise with GP practices, andthe aligned GPs, as well as with other health and social care providers,residents and relatives.

Rushcliffe PCN Care Home Team

HOURS -22.5 hours per week

Accountable to -Care Home Clinical Lead

PURPOSE OF THE ROLE

The purpose of this role is towork closely with the GP to provide clinical consultations, assessment and developclinical management plans for older people living in residential care withinthe Rushcliffe PCN.

This role will utilise youradvanced clinical skills as autonomous practitioner. You will lead the deliveryof regular care home rounds in the Rushcliffe Older People residential andnursing homes. Providing clinical management of care homeresidents presenting with acute and non-acute medical needs, undifferentiatedand long-term condition presentations. You will be developing a management planfollowing robust clinical consultations including assessment, examination,investigation, diagnosis, care planning, treatment and evaluation of care.

You will foster good workingrelations with the care home staff make a difference to the quality of healthcareprovided to residents in Rushcliffe PCN care homes.

Support the delivery of theNational Enhanced Healthcare in Care Homes, Direct Enhanced Service Framework(DES).

Main duties of the Role

Our service operates Monday toFriday, with no late shift, nights, weekend or bank holiday requirement.

You will be undertaking wardrounds in the care homes, reviewing residents with complex, non-complex,acute, and chronic conditions, and providing a proactive care approach. Thiswill require clinical consultations, examination, investigation, diagnosis,care planning, treatment and evaluation of care.

Medications management is a keypriority of the service offer. An an independent prescriber you will beundertaking regular medication reviews, interim prescriptions requests, developingcovert medication plans, prescribing, including covert medication panning and deprescribingas clinically appropriate. Liaising with the clinical pharmacists based in GPpractices as required.

Complete ReSPECT documentation,in conjunction with residents and relatives to support end of life planning. Residentsand relatives will be at the centre of decision making to ensure clear, conciseand informed wishes are documented and communicated.

As part of the EnhancedHealthcare in Care Homes enhanced service, this role will also support thedelivery of the specification. Using the CGA approach to new residentassessments, post resident reviews, and working with the care home and GPpractice to provide enhanced healthcare within a care home environment.

You will deliver a high standardof patient care with in-depth theoretical knowledge and evidence-basedpractice, working autonomously and with the support and supervision of aClinical Lead GP, as well as the care home aligned GP.

You will support the delivery ofCOVID and flu programme, by clinically screening and vaccinating care home residentsas part of the Rushcliffe and National vaccination programme.

As a senior clinician, you willprovide clinical leadership and expert professional advice to patients,relatives, carers, and colleagues to support the delivery of high-qualityclinical care, and this will, at times involve teaching, and education of CareHome or other staff.

The post holder will work acrossorganisational boundaries, fostering good relationships between providers andstatutory bodies and improve care for people living in long term residentialand nursing care.

As a registered AdvancedClinical Practitioner, you will be supported with a GP clinical lead andPartnersHealth to continually develop ACP knowledge and skills, in conjunctionwith the University and Nottingham Training Alliance (NATH) and othereducational organisations.

This job requires the postholderto travel and have face to face consultations with residents in care homes,undertake remote consultations and discussions, liaise with GP practices, andthe aligned GPs, as well as with other health and social care providers,residents and relatives.

ADMINISTRATION

Ensure consultations aredocumented accurately, clearly, and contemporaneously, in a timely manner.

Work with clinical ledgers toensure good time management and efficiencies.

Update patient records inaccordance with PCN policies and procedures meeting the requirements ofprofessional bodies NMC/HCPC. You will support delivery of the metrics requiredwithin the Enhanced Healthcare in Care Homes are met.

Ensure continuing education,training and development is undertaken to meet clinical governance guidelinesfor Continuing Professional Development and a Personal Development Plan.

Actively participate in clinicalnotes and prescribing audits, along with clinical supervision as part of theongoing CPD and governance compliance requirements.

Maintain professionalregistration with the NMC/HCPC as required to maintain the healthcareprofessional registration.

Provide evidence of a digitalbadge in Advanced Clinical Practice.

Undertake a fair share of theoverall administration associated with clinical practice within primary care,including answering the phone, supporting ledger management etc.

Be involved with clinical auditas part of new service implementation, quality improvement, patient reviews,QOF and any other audit work undertaken by the PCN Care Home Team and/orPartnersHealth

Show a caring and respectfulattitude towards patients, care home staff and colleagues. Have well-developedcommunication skills to provide treatment plans and work to a professional codeof ethics

Undertake and keep up to datewith relevant medical research, technology, and evidence-based medical practiceby attending continuing education courses and professional meetings, readingjournals etc.

As an employee ofPartnersHealth, working with patients registered with GP practices across thePCN, you will take due diligence and care regarding any information collected,recorded, processed or handled by you during the course of your work and that suchinformation is collected, recorded, processed and handled in compliance withPCN requirements and instructions.

The post holder will be requiredto travel as part of their duties to other sites in the PCN, and travelexpenses will be reimbursed by the employer.

To ensure observance andadherence to the Health and Safety at Work Act 1974.

To participate in staffmeetings, 1-2-1 meetings and the appraisal scheme and undertake trainingappropriate to the development of the job role and in line with the PCNsstatutory obligations.
