PCN Care Coordinator for Trinity Surgery (Part-time)

WisbechPrimary Care Network (PCN) is recruiting a CareCoordinatorfor Trinity Surgery to support patient care in the development of the Primary CareNetwork. This is a part-time role (20 hrs per week).
The appointed Care Coordinator will primarily be based at Trinity Surgery. On occasions, there may be a requirement to work at other sites within the PCN, namely Clarkson Surgery, North Brink Practice and Parson Drove Surgery.
Main duties of the jobThe Care Coordinator role is a critical and an evolving post with overall responsibility to help patients access the right services first time and will include:
To provide a point of contact for patients and act as a focal point for communication between patients, doctors and other medical staff
To provide efficient, accurate administrative support for clinicians, patients and colleagues.
About usThe PCNhas a population of approximately 50,000 patients and comprises of 4 PracticesTrinity Surgery, North Brink Practice, Clarkson Surgery and Parson DroveSurgery. The post holder will work alongside clinicians and other staff at Trinity Surgery as part of a multi-disciplinary team.

Job responsibilitiesReception Duties
Respond to all queries and requests for assistance from patients andvisitors
Greet patients and direct them from reception into waiting areas
Ensure patients without appointments but in need of urgent consultationare seen according to surgery protocol
Explain practice arrangements and formal requirements to new patientsand those seeking temporary cover; ensure procedures are completed
Advise patients of relevant charges for private services, accept paymentand issue receipt for same
Enter on computer any requests for home visits and provide a computerprint-out
Ensure that any prescriptions requested urgently for the day are passedfor action to the repeat prescription clerk or other person as appropriate
Action requests for ambulance transport
Ensure all incoming and outgoing mail is directed appropriately
Ensure reception and waiting areas are kept neat and tidy
Take fees from patients, entering in receipt book and issuing receipts
Ensure sufficient forms and documents are available in Reception
Work safely at all times in accordance with practice policy
The performance of any other reasonable duties requested by the Doctors,Practice Manager, Reception Team Leader or PCN Manager
Operation of the Telephone System
Receive and make calls as required in accordance with surgery protocol
Divert calls and take messages in accordance with surgery protocol
Ensure the system is operational at the beginning of each day andswitched over to night service and answerphone operational at the end of theday
Management of Appointment System
Ensure total familiarity with all appointment systems in effectincluding regular and incidental variations
Ensure appropriate booking of appointments and recalls in line withpractice protocol
Signpost patients to appropriate providers following practice protocols
Monitor effectiveness of the system and report any problems orvariations required
Administration Duties
Follow practice protocols to actions tasks and emails as appropriate
Provide administration support to clinicians, patients and practicemanagement
Start and End of Day Procedures
Open up premises at the start of each day, switch off alarm system andmake all necessary preparations to receive patients
Follow End of Day procedures and ensure preparations for the nextworking day are complete
Secure premises at end of day, ensure building is totally secure,internal lights off and alarm system activated
Any responsibility identified duringthe course of the job. This is anevolving role, and the candidate must be flexible and adaptable.

Person Specification Other requirements- Ability to travel and work across multiple sites with use of own car (business use included on insurance policy)
- Ability to work at other locations including home working if required
- Availability to work an 8.00am start time and 6.30pm finish time
- Willingness to work flexibly within the team to cover annual leave or sickness
- Ability to travel within the requirements of the role
- Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions
- Access to own transport insured for business use and ability to travel across the locality on a regular basis, including to visit people in their own homes
- Full driving licence with no restrictions and including business insurance cover
- Ability to work with patients on a 1:1 basis
- Can organise and prioritise workload
- Knowledge of IT systems, including ability to use Word, emails and the internet
- Awareness of GDPR
- Excellent written and verbal skills
- Effective communication, inter-personal and organisational skills, face to face, in writing and over the telephone
- Ability to work effectively as part of a team or individually as required
- Able to manage and prioritise own workload
- Able to work accurately with excellent attention to detail
- Experience with SystmOne clinical system
- Awareness of Safeguarding Children and Adults
- Experience of working in a busy administration role, working with minimal supervision
- Experience of establishing work processes and procedures
- Experience of working within a secure and confidential environment
- Experience of working in a healthcare setting
- Experience of inputting accurate and timely data onto electronic systems
- Educated to GCSE standard or equivalent
- Good command of written English
- NVQ qualification level 3 or above in Business Administration or equivalent previous experience
- Demonstrable experience of team working
- Ability to withstand sensitive and potentially stressful situations
- A positive attitude and helpful nature
