Older People's (OP) Associate Mental Health Practitioner

The closing date is 24 September 2025

This role will be working within the Older people's Community Mental Health Team within Bradford. The post will require the suitable candidate to be able to access a wide Bradford area due to a mixture of seeing patients at clinics and community settings.

Access to transport will be an asset due to the carrying of the physical health monitoring equipment.

Main duties of the job
  • To support the Community Mental Health Team (CMHT) in undertaking detailed assessments of ability for service users accessing the team.
  • To undertake physical health assessments and reviews of services users and promote healthy living and lifestyle.
  • Support the delivery of therapeutic Interventions.
  • Working alongside service users, family members and carers to provide signposting, support and advice.
  • Working in an interdisciplinary way to provide person-centred holistic care and promote independence for individuals.
  • To contribute to the Memory Assessment and Treatment Service (MATS) with ongoing evaluation, data collection and service development.

Person Specification Qualifications
  • NVQ level 3, foundation degree or postgraduate study
  • Postgraduate in a relevant field (e.g., Health & Social Care, Psychology Graduates)
Experience
  • Can demonstrate understanding and experience of working with diverse communities & families.
  • Ability to promote and develop links with stakeholders and communities.
  • Experience of working with older people
  • Experience of undertaking cognitive and functional assessments or working as part of an allied health professional team.
