Job Description
Great Oaks Hospice is a voluntaryorganisation that seeks to provide a range of End-of-Life Care services, freeof charge, for adults in the communityliving with a life shortening illness and to offer supportto their families, carers and friends. The purpose of the Hospice at Home Nurserole is:

To support the Hospice at Home Lead in the day-to-day coordination of a high qualityand integrated nursingservice that will enablepatients with life-limiting illnesses to be cared for at home, and to die athome if that is their preference

To assist in the ongoingdevelopment of the Hospice at Home Service

To assist in setting the required standardsof care and monitoring that these standardsare achieved

To assist in the recruitment, training and supportof Hospice at Home staff

Plan and organiseown day-to-day workload

Make decisions to enable comprehensive and coordinated supportof patients living at home

Main duties of the job Within the hospice to:
  • Follow policies and guidelines as per Staff Employees Handbook
  • Carry out responsibilities to best of your ability whilst also recognising and reporting limitations of own professional competence/experience
  • Recognise the need and undertake any training identified via Line Management and/or annual appraisal
  • Be aware of risk to self and others under Health and Saftey Regulations
  • Verbally report any accident or untoward incident as soon as possible followed by written report on teamnet
  • Respect people's personal choices for lifestyle and living arragements

About us Great Oaks Hospice is located in Coleford, Gloucestershire. We are a charity and our team provide end of life care services, free of charge, for adults, and their families, across the Forest of Dean Community.

Job responsibilities Clinical Responsibilities
  • To work a minimum of two - three Clinical Shifts per week,times as per service need.
  • To participate in the On Call Rota and potentially work unsociable and bank holidays asrequired.
  • To support the coordination the day to day runningof the service.
  • To ensure the safe and effective deliveryof care to those referredto the service.
  • To complete assessments, risk assessments and ongoing assessments for those on the Hospiceat Home Caseload.
  • To ensure the referral processis followed and that Care Needs Requestsare in place for those receivingHospice at Home Care.
  • To ensure, I collaboration with the Hospice at HomeLead that the staff rota is up todate and reviewed. To be proactive in filling any gaps on the rota either byfilling gaps yourself or by sourcing alternative cover.
  • To monitor and triage referrals appropriately and inany event within four hours of referral (or at the first available opportunityif working a clinical shift causes a delay.
  • To ensure that the patients recordsare accurate, up to date and maintained.
  • To ensure all visits are uploaded to Systmone promptlyto ensure staff are fullyup to date.
  • To effectively plan, prioritise and allocate resourceson a case-by-case basis.
  • To induct new staff and work with existing staff to ensure competence and enhance knowledge.
  • To support the allocation of resourceson a needs led basis.
  • To support the Hospice at Home Leadand act as a service coordinator for patients, carers, and health care professionals as required.
  • Along with the Hospice at Home Lead tobe the first point of contact for incoming calls and queriesfor the service.
  • To maintain an awareness of the Hospice at Homecaseload, including referral, admission, and discharge.
  • To agree treatment and care plans with referrerand Hospice at Home staff that reflectthe total needs of the patient.
  • Establish and maintaineffective communication with staff, patients,relatives and carersas part of their receipt ofpalliative and/or End of Life care.
  • Establish and maintain communication with individualsand groups about difficult or complex matters overcoming any problems in communication.
  • To participate in regular meetingswith other membersof the multi-professional team to implement and monitor the patientplan.
Leadership Responsibilities

  • To support theHospice at Home Lead with team meetings andreport to the Hospice at Home Leadas required.

  • To assist the Hospice at Home Lead in the development and implementation of evidence-based practice guidelines.
  • To promote the Hospice at Home serviceboth within GreatOaks Hospice and the local community.
  • To assist the Hospice at Home Lead in the development of strong links between theHospice at Home, Primary Health Care Teams, Local Authorities, and othercommunity agencies in order tooptimise the provision of service andbenefits for patients, their families and carers.
  • To support the monitoring and sign off timesheets andexpense forms, liaising with the Hospice at Home Lead as appropriate.
  • To assist the Hospice at Home Lead in the audit and evaluation of the Hospiceat Home Service.To work effectively with allpersonnel within the Hospice at Home team, hospice multi-disciplinary team,community staff, social services, hospital professionals and the specialistpalliative care team.
  • To maintain regularprofessional contact with other registered nurses within the hospice, the specialistpalliative care team and within other local pertinent organisations e.g. primaryhealth care teams and the NHS PCT.
  • To support the monitoring of the quality of the workwithin the Hospice at Home team and alert the Hospice at Home Lead to qualityissues.
Professional Responsibilities
  • To remain aware of the Nursing and Midwifery CouncilStatutory Requirements for Practice, Guidelines and Codes of Practice.
  • To be professionally accountable for all aspectsof own work, and to understand the need for professional self-development.
  • To comply fullywith Great Oaks Hospice Operational and Clinical Guidelines.
  • To ensurepatient and servicerecords are recorded and maintained in accordance with Great Oaks Hospice Policies and professionalstandards.
  • Effectively comply with all NMCregulations.
  • Ensure own actionspromote quality.
  • To understand own role within the organisation and Hospice at Home team.
  • Work within the current clinicalgovernance framework and maintain appropriate policies andprocedures/guidelines and protocols as designated by Great Oaks Hospice.
  • Undertake Mandatory/Statutory trainingas provided by Great Oaks Hospice.
  • Undertake regular Line Management and supervision in support ofcontinuing professional development and resolution of personal reactions to the accumulative emotional nature of the job.
Other Responsibilities
  • Act as a responsible individual in relation to Health and Safety and be aware of the responsibilities of allemployees to maintaina safe and healthy environment for patients, visitors,staff, and volunteers
  • Ensure that all duties are carried out to the highest standard, and in accordance with current quality initiatives within the work area.
  • Undertake flexible hoursincluding additional clinical shifts,on-call duties, antisocial hours as and whenthe need arises ensure staffing levels are adequate.
  • Ensure that the disclosure and use of confidentialstaff information is both lawful and ethical, and to recognise ownresponsibility for compliance with relevant legislation.
  • Promote, at all levels, the Hospice's vision,values, and strategicobjectives.
  • Hold an enhancedDBS and Occupational Health clearances appropriate to the role.
Person Specification Qualifications
  • Registered Nurse (Level 1&2) with minimum of Band 6 level of competence and experience
  • Evidence of professional development within the field of palliative and End of Life Care
  • Recognised qualifications in any of the following: -
  • Diploma or degree in palliative care or related nursing subject
Knowledge and Attributes
  • Knowledge of the GSF and LCP in palliative care
  • Knowledge of current symptom management of including that of syringe drivers
  • Knowledge of Advance Care Planning
  • Clear and concise documentation skills
  • Ability to organise and prioritise workload
  • Ability to create effective and safe solutions to unexpected problems
  • Sound clinical skills
  • Excellent communication and negotiation skills, ale to communicate effectively at all levels
