Neurodiversity Family Support Practitioner

In this role you will have the opportunity to work closely with colleagues, children and their families to help us to better understand the challenges they face when children have additional needs.
You will be delivering a range of information, advice and guidance for children, families and practitioners. You will be central to improving the way in which we support children, families and practitioners to navigate the many challenges and successes of being neurodivergent.
Using your skills and knowledge, you will ensure that practitioners across the workforce are able to map a child's individual needs and plan effectively to meet these in a timely way. You will ensure information, advice and guidance is provided to children, families and practitioners in the most effective way to ensure that improved outcomes are achieved, including reducing demand on high-cost specialist services.
Main duties of the jobYour daily tasks will be varied and include:
- Delivering training and resources to support a more inclusive and adaptive school environment that celebrates neurodiversity through use of a tool that enables identification of individual needs.
- Working in partnership with schools to provide additional support and guidance
- Carrying a caseload of families and work with key agencies (such as PCN Navigators) to ensure families are able to access the support that meets their needs
This is a busy role so you will be working together within your team to balance different tasks that often change. You will need a full driving licence to be able to travel to the different sites across the community and schools.

Person Specification Qualifications- Additional Qualifications and Training
- Experience of working with ND young people
- experience of using IT systems / case notes
- Experience of mentoring others
- A good knowledge and understanding of local and national frameworks relevant to social models of disability/SEND and its application across relevant systems in education and healthcare.
- Sound communication skills
- Customer focused approach, for working with young people. Representing the trust. Working in a multi agency way. E1
- Sounds knowledge of safeguarding and when to elevate
