Mental Health Practitioner

Overview

Are you ready to take on an exciting role working in Adult Mental Health on a full time basis with the New Forest CMHT (East) team based at Anchor House in Totton.

If you're innovative, proactive, and passionate about providing high-quality holistic care, then we would love to hear from you. You will have a broad depth of mental health experience working with individuals who have severe and enduring mental illness.

Responsibilities

• You will be responsible for care-coordinating a caseload of service users with complex long-term mental health problems who find it difficult to engage with services.
• You will have experience in assessing and formulating individual intervention plans and outcome measures for adults with complex mental health problems and have current knowledge of NHS policies and practices in mental health.
• You will work in a strengths-based and recovery-focused way to support service users to meet their mental health needs.

Qualifications and experience

You will have a broad depth of mental health experience working with individuals who have severe and enduring mental illness.

Eligibility

Only open to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service, Isle of Wight NHS FT or Hampshire Hospitals FT.
