Mental Health Practitioner - MHIST- Rehab Access

An exciting and unique opportunity to join the Mental Health Intensive Support Team within the Rehab Access Function, supporting the Clinical Lead in coordinating rehabilitation and out of area assessments.

• Formulate specialist assessments for individuals requiring rehabilitation in community and inpatient settings.
• Deputise for the Clinical Lead during periods of leave, sickness and absence.
• Act as the point of contact for all internal and external agencies involved in the Rehab Access function.
• Screen referrals and liaise with professionals, families and carers in partnership with the Clinical Lead.
• Complete assessments and formulate rehabilitation pathways.
• Liaise with inpatient services in acute care to provide specialist advice on rehabilitation and support discharge planning.
• Signpost service users and carers to appropriate services to meet individual needs.
• Deliver evidence based interventions that are measurable and promote the service user's journey along the recovery pathway.
• Attend weekly Gatekeeping meetings to discuss assessments with the Rehab MDT and external funding bodies.
• Supervise junior staff.
