Mental Health Clinician

Working collaboratively with patients and their families/carers, the postholders will have a particular focus on delivering high intensity and time limited community interventions aimed at providing an alternative to mental health acute/crisis admissions and to facilitate early discharge, reablement and mental health recovery.
This involves supporting team core tasks, telephone triage, support and signposting and face to face full needs assessment care planning and referring on to relevant services.
Postholders will also work alongside inpatient staff, clinical leads and community partners, to further develop Rotherham's OPMH crisis/home treatment and inpatient discharge pathways.
These positions require a high level of autonomous practice and practitioners will be confident clinicians able to make competent decisions about risk, care planning and the appropriateness of home treatment. Postholders will have relevant knowledge and experience of mental health acute/ crisis work in the community.
The MDT team includes nursing, OT, psychology, support and medical staff. The work of all team members is valued and staff are encouraged to participate in CPD & career progression.
Postholders will be supported by regular supervision and applicants from a range of professions, including nursing, occupational therapy and social work are welcomed.
Main duties of the jobDeliver high-quality, person-centred clinical service for older adults with mental health needs, within a multidisciplinary team. Specialist assessment, care planning, intervention and evaluation for planned and unplanned care, with a strong focus on supporting people in their own homes/community settings. Practice will be compassionate and evidence based, addressing complex mental health presentations alongside cognitive and physical health needs.
Use a wide range of clinical and therapeutic skills to develop individualised care and crisis plans, crisis management and short-term biomedical, psychosocial and multidisciplinary interventions. Coordinating care with health, with social care and partner agencies, support safe and timely discharge, be a specialist resource to GPs, primary care teams and other professionals.
Assessment of mental health needs and risk, implementation and evaluation of care plans with service users and carers, and provision of carer support/signposting. Effective communication, record keeping and discharge planning are essential to ensure continuity of care and safe transitions between services.
Adhere to safeguarding procedures, Mental Health Act and Trust policies, clinical governance, audit, service development and research, care co-ordination and Section 117. Utilise supervision, appraisal and PDR, support students, and may deputise for senior staff when required.
Person Specification Qualifications- Registered Occupational Therapist HCPC registered
- Mental Nursing Certificate (RMN) Registration on NMC register.
- Appropriate demonstrable experience of working with older people with mental health problems and / or adults with complex emotional needs / personality disorder
- Knowledge of appropriate legislation e.g. current Mental Health Act legislation, safeguarding legislation and other policy related to mental health
- Excellent assessment and analytical skills
- Ability to work as part of a team and on own initiative.
- Evidence of substantial experience as a qualified clinician.
- Mental health care/treatment relevant to service.
- Multi-agency working /working across service interfaces within a health and social care setting.
- Experience of assessing and managing risk
- Evidenced based clinical interventions relevant to the service and role.
- Relevant specialist clinical courses/training.
- Models of care and treatment relevant to working with people in mental health crisis
- Evidence of continuing professional development.
- An understanding of current mental health disorder / issues.
- Up to date clinical knowledge including risk assessment, dual diagnosis and complex health and social care needs.
- Assessing, formulating, and working with people & families presenting in heightened state of distress and trauma.
- Ability to use initiative and work without immediate supervision
- Ability to demonstrate an understanding, empathic approach
- Clinical risk assessment and contingency planning.
- Access to transport and the ability to travel between the required geographical area.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
£39,959 to £48,117 a year per annum, pro rata
