Position SummaryThe Mental Health & Wellbeing Practitioner will provide support to people who have encounters with the Justice System in Angus. Working in partnership with Community Justice Services and other local partners, you will help individuals with complex and co existing needs related to mental health, poverty, domestic abuse, care experience, homelessness and other social barriers to progress in their recovery and lead a meaningful life in the community.
Key Responsibilities- Deliver person centred support and interventions for individuals accessing justice services.
- Collaborate with Community Justice teams and partner agencies.
- Facilitate recovery and wellbeing planning, including assessment, goal setting and progress monitoring.
- Advocate for clients' rights and needs within the justice system.
- Maintain accurate and confidential case documentation.
- Participate in multidisciplinary team meetings to improve service delivery.
