Memory Clinic Nurse - Memory Assessment Service

We are looking for a Registered Mental Health Nurse (RMN) to join our Memory Assessment Service, in our Memory Clinic. You will have specialist knowledge of the care of older adults with mental health conditions and co existing physical health concerns.

The Memory Assessment service in Buckinghamshire has two main bases: Aylesbury and High Wycombe.

The team are friendly and supportive as well as dedicated to providing excellent care and service through evidence based interventions, research and quality improvement work. We work closely with families and carers as well as other services including social services who may be involved in supporting our patients/clients who have complex needs. The team enjoy having a range of students learning and working with them and have gained consistent positive feedback.

This role is part time working 22.5 hours per week.

Main duties of the job

This post will be within a well established team which will be working together to provide pre and post diagnosis support for service users with concerns about their memory and their careers.

The service will be delivered mainly in a clinic environment, but on occasions home visits may be needed. The aim of this service is providing early assessment and diagnosis of memory concerns and intensive post diagnostic support.

To provide a highly specialised nursing assessment, evidence based intervention, advice and support for their own case load of people with concerns around memory function or a new diagnosis of dementia and their careers. The post holder will provide up to date clinical advice, support liaison education and training to other staff and agencies.

Will be an active member of the multi disciplinary team and will work in close co operation with primary care and other community agencies.

Job responsibilities

The Trust provides many development opportunities for staff through our Learning and Development department and links with universities and our Quality Improvement team. Health Education England are also providing funding within the Trust, to support staff training needs. The leadership team are committed to continuous development and improvement through supervision, training, reflective practice, peer review and staff wellbeing.

The successful candidate should be enthusiastic, resourceful and caring, they should have excellent communication skills, both written and verbal. They will need to demonstrate a good awareness of mental health and knowledge of working with older adults with mental health problems. In return we can offer a full induction and a focus on developing their knowledge, skills and confidence further, in relation to all aspects of the post, as well as contributing to service improvements/development within the team. This is an excellent opportunity for the successful candidate to develop their career plan further.

Applicants will need the ability to travel across the area to see patients within their homes, in a location where there are many villages and limited public transport to some of these areas. The ability to travel independently between sites within the Trust is essential for this role.

Person Specification Education
  • Professional qualification as RMN or OT
  • Postgrad training in research or Non Medical prescriber
Knowledge
  • Experience of working with older adults in a mental health setting
  • Experience of working in a memory clinic
