Telephone support for those with long term health conditions in Hull and East Riding

Can you help people with long-term health conditions in Hull & East Riding?

Hull CVS Health Captains are looking for volunteers for our Long term Health Conditions Buddy Service, to help others through their care journey.

If you have experience of living with a long term health condition or have cared or supported others with the same, we would like to hear from you.

The role will involve offering a friendly ear to provide practical support and advice over the telephone to those who are living with a long term health condition.

Days and times are flexible and can work around you and there is also options to volunteer from home.

Tasks and responsibilities

• Calling on the phone and checking on the clients wellbeing.
• Offering your lived experience to provide peer support to others.
• Escalating any healthcare/ social care needs when needed.
• Clear and accurate record keeping and submitting of case notes to project lead.
• Signposting clients to appropriate and helpful information, services and support groups.
