Overview

Locum GP required for an ongoing position in a GP surgery setting based in a idyllic town in South Somerset on the Dorset Border. The practice is a well-established, friendly, patient-centred practice close to the Jurassic Coast with beautiful surrounding countryside. It is a semi-rural practice based in a Somerset market town with purpose-built premises and an in-house part-owned pharmacy. This role will be based primarily at one site, but there will be a possibility you will be required to do some sessions at a sister site located 0.2 miles away (3 minutes on foot or 2 minutes by car). The practice benefits from a high quota of nursing and support staff including 2 nurse practitioners, 5 nurses, 1 ECP, 5 HCAs. The two sites jointly service a patient list of around 11,000. The role is for a GP looking to work full-time Monday to Friday on an ongoing locum basis, with the option to work a couple of regular days per week if preferred.

Locum GP required for an ongoing position in a GP surgery setting based in Somerset. Great rates hourly or day.

Duties

Duties will include:

• To proactively manage the health of the patient list with the support of the EPC team (including health coaches, care co-ordinators, advanced nurse practitioners, social workers, mental health workers etc.), with emphasis on patients where there are clinical concerns.
• To conduct face-to-face consultations with patients requiring GP input.
• Compiling and issuing computer-generated acute and repeat prescriptions (avoiding handwritten prescriptions whenever possible).
• Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.
• To provide extended appointments and/or home visits for patients where this need has been identified.
• To act as a clinical liaison providing feedback/information on practice patients who are being supported through EPC and the Complex Care teams while working closely with hospital consultants, community agencies and the voluntary sector to find new ways to support patients.
• To record clinical information in the GP practice computerised clinical software systems.
• The GP would be able to help "mould" their role by providing regular feedback to the EPC working group and SHS to help develop the job in a way that best suits the EPC model. They would also have a role in teaching/training the wider practice team, including both clinical and non-clinical colleagues.
• To adhere to the professional standards as set out by the GMC.
