Highlights of Your Role Case Management

• Works in consultation with youth/placing agency/ parents/ therapist/ psychiatrist/ school/ child welfare.
• Creates specialized ministry mandated plans of care (PoC) with individualized goals for a maximum of 4 individual clients.
• Evaluates treatment goals utilizing approved measurement tools for the purpose of tracking client progress.
• Utilizes a trauma-informed approach which is client-centred and works within an anti-oppressive framework and equity lens.
• Completes ministry mandated risk assessments and safety plans in accordance with ministry guidelines and YouthLink's policies and procedures.
• Contributes to the onboarding and orientation process for all new staff, students and volunteers and contributes to their learning journey.
• Communicates with the team regarding treatment plans for youth, goals identified, medical treatments/diagnosis, and updates any required medication changes.
• Contributes to the intake orientation process which includes policy review.
• Provides a tour of the treatment home and surrounding area, reviews program requirements with placing agency and ensures all required documentation is signed and understood.
• Ensures medical care is provided at regular intervals as per ministry guidelines and YouthLink's policy and procedures.
• Connects youth to outside resources to support their treatment goals and transition planning.
• Advocates for appropriate educational placement and resources. Ensures youth receive the needed educational supports, such as assessments, IEPs, and any other related educational resources/tools required to succeed.
• Attends parent-teacher interviews to stay current with student progress and any identified challenges. Follows up with youth and educational supports to collaborate and ensure any identified concerns are addressed.
• Schedules one-to-one meetings with assigned youth to build meaningful relationships which support overall PoC. This includes scheduled outings, activities, and intentional conversations which enhance youth engagement.
• Supports LIT Youth Workers in teaching self-care and good hygiene practices, room cleanliness, personal safety, boundaries etc.
• Collaborates with LIT Youth Workers to celebrate and embrace youths' identity: race, ethnicity, spirituality, ability, gender identity and sexual orientation.
• Participates in trainings that align with ministry framework, YouthLink's strategic plan, program requirements and treatment modalities.

Plan and Review

• Develops individualized POCs, utilizing information obtained from the pre-admission intake, meetings with the client (includes all contacts), reports written by staff and other service providers at each interval.
• Assesses needs, strengths, identity characteristics, services and treatment, adult ally/resource person, safety planning, education, meals and nutrition, transfer and discharge and identification of goals and goals achieved.
• Ensures risk assessments are completed at each POC/ROC interval according to ministry timelines (30-day, 90-day, 180-day and every 180 days thereafter).
• Reviews all YouthLink policies and procedures related to the treatment home, which includes rights, responsibilities, complaint procedure, residential placement advisory committee, ombudsman, mechanical restraint, physical restraint, and placement program description related to cultural competency.
• Facilitates PoC meetings with youth, placing agency and other identified supports as per ministry timelines and YouthLink's policies and procedures.

Direct Client Care

• Acts as a role model and teaching emotional regulation skills.
• Supports LIT Youth Workers by ensuring routine and structure are maintained throughout the day: wake ups, day program activities, reminding of appointments, checking in with youth throughout the day.
• Provides mediation for any peer and family dynamics that may arise.
• Implements crisis management practices in accordance with ministry guidelines and YouthLink's policies and procedures. Conducts risk assessments and safety planning as required.
• Contributes to therapeutic-based programming such as life skills, peer support, house meetings, physical activities and tutoring. In addition, provides experiential learning outdoors.

Meetings, Committees and Collaborative Networking

• Attends weekly case management and monthly CoP meetings and presents assigned clients for team discussion and treatment planning. In addition, attends bi-weekly business team meetings focused on program administration, agency and ministry updates and strategic planning.
• Participates in required agency committee such as health and safety, ABR etc.
• Attends employment fairs, school and agency events and participates in agency initiatives.
• Accompanies youth to their psychiatric, medical and therapy appointments

Report Writing and Health Record Management

• Ensures accurate and timely data entry into digital database and hardcopy files as per ministry guidelines, accreditation and YouthLink policies and procedures.
• Completes regularly scheduled file audits in accordance to ministry standards, accreditation and YouthLink policy and procedures.
• Enters client contact notes (recording indirect/direct) in compliance with ministry standards and YouthLink policies and procedures.
• Prepares Ministry of Health Serious Occurrence reports, internal incident reports, and files missing person reports when appropriate.
• Medical tracking sheets, medical reordering, medication dispensing and documenting, completing medication audits.
• Utilizes EMHware to book client appointments.
• Dispenses medication if needed as a backup, keeps accurate and timely records and completes medication error report when appropriate.
• Keeps accurate records of petty cash and TTC/presto cards.
• Records achievements made by youth throughout the week to foster resiliency and recognize small successes

Other duties as assigned. Highlights of What You Bring Education

• Diploma in Child and Youth Work, Social Service Worker, Social Work or Psychology or relevant field, and/or a combination of equivalent education and experience.
• Bachelor/Master of Social Work Bachelor of Psychology/Sociology, Bachelor/Master of Education (Counselling Psychology) is an asset.
• Specialized training in attachment theory, developmental lifespan stages, neurobiology of the brain and the impact of trauma is an asset.
• Specialized training in relevant interventions to address behavioural issues from a trauma informed attachment lens is an asset.

Experience

• 2 years working with youth with complex needs in a mental health setting.
• Experienced in providing care utilizing a trauma-informed lens.
• 3 years working with youth with complex needs specifically in child welfare and/or Live-In Treatment.
• Experienced in providing care utilizing a trauma-informed lens.

Knowledge, Skills and Abilities

• Knowledge of community services, relevant CYFSA legislation, and social justice issues.
• Experience with suicide/risk assessment and crisis support services.
• Understanding of adolescent development, trauma responses, attachment theory, and evidence based interventions.
• Experience implementing therapeutic routines in a live-in treatment setting.
• Ability to lead or co facilitate therapeutic groups.
• Strong interpersonal, conflict resolution, and organizational skills.
