Job Description
The Lausanne Health Network is an association that brings together more than a hundred institutions and health professionals - CHUV, treatment centers and nursing homes (EMS), home care (CMS) and doctors - as well as around a hundred partners. We contribute to healthcare access and the continuity of care, particularly through the Regional Office (BRIO). We also carry out projects aimed at improving the quality and continuity of care, mainly for the elderly, who are vulnerable due to one or more chronic illnesses. Following a retirement, we are looking for: Liaison Nurse.You enjoy working independently while creating relationships? You are sensitive to supporting vulnerable elderly individuals and their relatives? You want to engage in a dynamic activity while working with and in different care environments? Then you are the person we are looking for!Your mission:Anticipate and plan for the user in the care network according to their needs and in collaboration with other professionals, taking into account their needs and the resources of the system. Organize both short and long stays in EMS and assist with the necessary home care upon return. Facilitate and coordinate the continuity of care by communicating and acting as an interface/pivot among different partners. Inform and propose options to the user, family caregivers, and professionals based on the evolution of the situation.Profile:Nursing diploma (Bachelor HES or equivalent). Professional experience in hospital, community settings, and good knowledge of the Vaud socio-health network. Skills to work with family, medical, and institutional networks in an innovative context, with strong listening and analysis abilities, proficiency in standard office software, and ease in analysis, synthesis, and management of priorities.We offer you:A varied activity in a constantly evolving public interest field, within a motivated and dynamic team.
