Leaving Care Personal Advisor - Pathway & Independence Coach

Schools' Choice in Shrewsbury is seeking a Personal Advisor within the Leaving Care Service to manage a complex caseload of young people and support care leavers, including asylum-seeking youths and those with challenging immigration statuses.

You will undertake needs assessments, develop SMART pathway plans, promote independence, and collaborate with multiple agencies to secure positive outcomes and enduring support networks for the care leaver.
