Lead Worker 
Telford and Wrekin
£19,827.60 increasing 1st May £20,982.00 per annum
30 hours per week
About the roleAs a Lead Worker, you will provide early, targeted, and flexible support to individuals and families across Telford and Wrekin. You will work with young people aged 16-54 and older individuals aged 55+, supporting them to maintain their tenancies, achieve financial independence, and improve their health and well-being. This role focuses on homelessness prevention, tenancy sustainment, and promoting independent living.
What you'll be doing- Conduct comprehensive assessments of customers' housing, financial, health, and social support needs.
- Develop personalised, outcome-focused support plans tailored to individual strengths and goals.
- Collaborate with multi-agency partners to deliver coordinated and holistic support services.
- Provide practical assistance with budgeting, debt management, and accessing welfare benefits.
- Support customers in connecting with community services, reducing isolation, and fostering social inclusion.
- Uphold safeguarding standards, ensuring the health and safety of vulnerable individuals.
- Maintain accurate records, ensuring compliance with Key Performance Indicators (KPIs).
- A motivated and compassionate individual, driven to make a positive impact on the lives of vulnerable people.
- Ability to work autonomously and as part of a team, demonstrating accountability and professional integrity.
- Strong communication and advocacy skills, with experience working collaboratively with external agencies.
- Proficient in IT systems and record-keeping, ensuring accurate documentation of customer support.
- Knowledge of housing-related support, financial management, and safeguarding practices.
- Flexibility and adaptability in supporting a diverse caseload across multiple locations.
