Lead Primary Care Mental Health Triage & Interventions

Wight Healthcare is seeking a clinician to contribute to a triage service, offering early assessment, brief interventions and signposting for people with mental health needs in local communities.

You will develop systems and processes to support care delivery, and work in partnership with primary and secondary care, including Solent Mind and peer recovery workers alongside practitioners in the surgery.
