Job Description

An exciting opportunity has arisen for a Lead nurse to join our established team and lead on the care of the COPD Asthma patient cohort and work alongside our team to manage QOF work.

We are looking for someone who is highly experienced and passionate about diabetes management in a primary care setting. This role would be suitable for someone currently managing chronic diseases in a primary care setting looking to take the next step.

Main duties of the job

To provide a specialist COPD /Asthma nursing service to adultswith COPD and Asthma , delivering care, education and support to people nd their families/carers.

Be part of the leadership team responsible for definedworkstreams and outcome measures ensuring expertise in practice and clinicalleadership through collaboration, vision, education, research and practicedevelopment including further work with chronic disease management.

Job responsibilities

The successful candidate will have proven experience within a primary care setting and active knowledge, experience and interest in COPD /Asthma management.

Education Role

  • Work alongside our well being co-ordinator to deliver education to our patients in the self-management of the chronic diseases .
  • Maintain links and communication with the patients, carers and relatives around condition management.

Clinical Role

  • To facilitate specifically designed and designated clinics based at Park Lane Practice and Phoenix Surgery.
  • To demonstrate advanced knowledge of COPD /Asthma and other chronic diseases and their co-morbidities using appropriate skillsand innovative practice to ensure the delivery of equitable, quality, safe care that is up to date,evidence based and within local and national guidelines.
  • To undertake Respiratoy clinics within the practice, reviewing the care and management ofpatients interpreting laboratory results and the patients own self- monitoringresults to ensure accurate diagnosis, effective treatment and longer term management .
  • To make decisions regarding the diagnosis, initial and on-going treatment, management andprescribing for people with chronic Diseases .
  • To liaise with other health and social care professionals or voluntary organisations, whereappropriate, both hospital and community based.
  • To work autonomously as an independent practitioner, demonstrating expertise in theassessment, prescription implementation, planning and evaluation of patient care.
  • Work alongside a multi-disciplinary team to actively plan, manage and undertake QOF work within the practices.
Person Specification Qualifications
  • Registered General Nurse (Registered with the Nursing and Midwifery Council)
  • Recognised qualification in the management of Diabetes
Knowledge and Skills
  • Understanding of NICE guidelines and Respiratoy healthcare essentials
  • Understanding of Quality and Outcomes Framework
  • Understanding of medication and administration of medication relating
    to respiratory diseases and other Chronic Diseases
