Overview

We have an exciting opportunity for an Integrated Care Nurse to join our Planned Care team at HMP Pentonville, a category B prison located in London.

HMP Pentonville is a male prison with a capacity of 1,200 inmates; it is a remand prison for inmates awaiting trial. It is well situated in London and is 10 minutes from Kings Cross station.

There is an on-site staff canteen and gym, and the training and development opportunities are strong. This site is well invested in its staff.

Hours

37.5 hours a week, Monday to Friday, 7.30am-4pm. Some flexibility required.

This position is full time; the salary is up to £52,000 per annum FTE, depending on experience.

Main duties of the job

As an Integrated Care Nurse your responsibilities will vary. You will:

  • A Physical Health Nurse working in mental health settings plays a vital role in bridging the gap between physical and mental healthcare.
  • Assess and manage the physical health needs of individuals with serious mental illness, often collaborating with multidisciplinary teams to deliver holistic, evidence-based care.
  • Monitor vital signs, coordinate clinics, support staff training, and promote health awareness to reduce health inequalities and prevent diagnostic overshadowing.
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
  • Use your skills and knowledge to deliver nurse-led clinics to your patient group while being supported by a multidisciplinary team.
  • Provide outstanding evidence-based care to patients, seeking out, listening to and acting on their feedback so that care is personalised and informed by what matters to them.

Qualifications
  • Primary Care experience, Practice Nurse or Mental Health experience - desirable
  • Evidence of Continued Professional Development
  • Understanding of the complexities of multi-disciplinary and multi-agency working
  • A non-judgmental and compassionate approach

Although not essential, ideal applicants would have experience in a hospital, community or prison environment.
