Impactful Housing & Homelessness Case Worker

Location: Greater Western Sydney Salary: $70,000-$85,000 + Super

Crescent Care's Housing & Homelessness Assistance program is seeking a dedicated Case Worker to provide end-to-end support to individuals and families at risk of homelessness. You will work closely with clients to develop sustainable housing solutions and improve long-term outcomes.

Key Responsibilities:

• Conduct housing assessments and develop case management plans.
• Provide tenancy support, budgeting advice, and assistance securing long-term accommodation.
• Advocate on behalf of clients with housing providers and government agencies.
• Facilitate access to mental health, employment, and financial counselling services.
• Prepare detailed case notes and compliance reports.

Requirements:

• Diploma or Degree in Community Services, Social Work, or related field.
• Strong knowledge of the housing and homelessness sector.
• Experience supporting clients with complex needs.
• Excellent interpersonal and negotiation skills.
• Current driver's licence and reliable transport.
