Hospital Discharge & Care Hub Coordinator

Slough Borough Council is seeking a highly motivated Business Coordinator to join the Hospital Hub team, coordinating hospital discharge pathways and ensuring individuals receive the right support at the right time for safe and timely discharge from hospital.

Working with health and social care professionals, you will manage referrals, coordinate assessments and service activity, monitor performance data, and act as a central point of contact for partner organisations.
