Healthy Lifestyle Practitioner

The Healthy Lifestyle Practitioner will provide one to one and group-based interventions to support individuals to improve their health and wellbeing, including specialist advice on smoking cessation. The role empowers service users to identify how their behaviours affect health, set personal goals, develop action plans, and sustain long term lifestyle changes.

SUMMARY OF RESPONSIBILITIES AND DUTIES

• Provide tailored one to one and group stop smoking support.
• Support individuals to identify health needs and barriers.
• Take routine measurements.
• Deliver structured stop smoking programmes.
• Conduct follow ups and ensure continuity.
• Signpost service users to relevant services
