Role Purpose:

You will be a 'Healthy Lifestyle Advisor' for Stockport's Integrated Lifestyle Service Your Health Stockport, specialising in delivering high quality physical activity and contributing towards getting the residents of Stockport active through our interventions. You will also be required to deliver on our weight management curriculum communicating the key messages around behaviour change and sustaining a healthy lifestyle.

You will be a motivated, passionate, organised, and proactive advisor, with experience working in community health programmes. You will be passionate about promoting healthy lifestyles and delivering motivational change which is client centred. You will have a background in supporting people to build their confidence and ability to make sustainable healthy behaviour changes which works for their personal circumstances.

Our team have working backgrounds in nutrition, weight management, exercise, brief alcohol advice and smoking cessation. We offer you a full in-house training package to help you support our clients on their journey to a sustainable healthy lifestyle change.

Key Duties and Responsibilities:

Key duties and responsibilities include delivering group exercise sessions in line with weight management protocols and NICE guidelines, and facilitating ABL Health's adult and family weight management programmes within community settings. This includes delivering sessions on nutrition, physical activity, behaviour change, and exercise to both groups and individuals, while leading client-centred assessments and providing evidence-based education aligned with ABL Health programme curriculums. The role involves supporting clients using a client-centred approach, incorporating positive psychology, motivational interviewing, and SMART goal setting to encourage sustainable lifestyle changes. You will manage and monitor a caseload of clients, guiding them through appropriate lifestyle interventions and tracking their progress throughout their journey with ABL. Responsibilities also include maintaining accurate records and completing administrative tasks in line with organisational policies and procedures, ensuring all documentation is up to date and compliant. You will be expected to stay informed of current best practices and maintain relevant qualifications to ensure clients receive evidence-based support to achieve their health and weight management goals. The role requires a compassionate and empathetic approach when working with vulnerable individuals, helping them make positive, lasting changes to their health and wellbeing. Additionally, you will contribute to the ongoing development of programme resources, complete all mandatory and additional training, effectively organise your workload to meet priorities and deadlines, and actively participate in team and wider organisational meetings. Working collaboratively with a multi-disciplinary team, you will support individuals to improve and sustain healthy behaviours such as increasing physical activity, improving diet, reducing alcohol intake, and stopping smoking, while operating within the framework of clinical governance to maintain and enhance the quality, safety, and efficiency of care.

Skills and Competencies Required
  • Experience of implementing and delivering behaviour change interventions within a community health environment.
  • Positive and proactive in your delivery approach with the ability to motivate, engage and support individuals to achieve their targets and positive health outcomes.
  • Adept in communication; A strong communicator who adapts the message to fit the needs of the audience with excellent listening skills and positive approach.
  • Build strong relationships; Foster trust and cooperation among clients, colleagues, stakeholders and community leaders, to develop and sustain personal contact in order to benefit health and wellbeing of the people of Oldham.
