Healthcare Assistant - Community and Hospice Line

We are looking for a calm, compassionate Healthcare Assistant (HCA) with excellent communication skills to join our community nursing and hospice line team - a collaborative telephone support service for people approaching the end of life and their families.
Main Duties- Provide emotional support for anxious or distressed families
- Coordinate community responses
- Record changes in condition
- Take calls when a death has occurred
- Visit patients in their own homes
- A calm, confident communicator
- Compassionate and patient
- Experienced in a care or healthcare setting
- Able to make safe decisions and raise concerns appropriately
- Comfortable working independently in a community setting
- Reliable, resilient and confident using electronic systems
- Willing to attend daytime training and work flexibly
- Make a real difference to people's experience of end of life care
