Health & Wellbeing Coach

As a Health and Wellbeing Coach, you will empower individuals to take control of their physical and mental health by supporting behavior change and promoting self-management. Using health coaching, motivational interviewing, and behavior change techniques, you will help people build the knowledge, skills, and confidence to make positive lifestyle changes. This role takes a holistic approach, increasing access to community support and self-management education. Working closely with GPs, Care Navigators and other healthcare professionals within Primary Care Networks, you will deliver personalised care to individuals referred for support with conditions such as diabetes, obesity, hypertension, and stress, as well as those needing emotional and motivational support. Westminster is home to a very diverse population, and the residents you will work with may come from a range of backgrounds - some more challenging than others. This role is especially well-suited to someone with knowledge, experience, and enthusiasm in psychology, wellbeing, fitness, health, or nutrition.
Role Requirements and Responsibilities- Patient and Community Engagement
- Encourage patients, their families, and carers to actively participate in their own healthcare, facilitating access to community resources, peer support groups, and other wellbeing activities.
- Oversee the use of health technology to support patient engagement and lifestyle changes, such as tele-health and action planning tools.
- See patients on a one-to-one basis within GP practice clinics, as well as in groups providing health and wellness support.
- Work with patients over multiple sessions (up to 12) to coach, motivate, and support them in identifying their needs, setting goals, and putting their personalised health and care plan into action.
- Collaborative Working
- Build and maintain relationships with external agencies and local community services to foster a coordinated approach to patient care.
- Work closely within the PCN's multidisciplinary team (MDT) and with GP practices to facilitate appropriate patient referrals and data entry, adhering to data protection standards and agreements.
- Drive to embed themselves within their GP practices alongside other clinical type roles.
- Contribute to adhoc project work, including support to the Health Equity team, or other HCL colleagues.
- Documentation and Administration
- Ensure accurate and up-to-date patient records, including documentation of all health coaching interventions, referral processes, and directory of services.
- Support practice staff in patient referrals and identification of individuals suitable for health and wellbeing coaching.
- Present health education in both one-on-one and group settings, including facilitating discussions and action plans that empower patients to make informed health choices.
- Proven experience in health coaching, motivational interviewing, and behaviour change techniques.
- Experience within a healthcare setting, with a strong focus on patient-centred care.
- Excellent communication and interpersonal skills, with the ability to build effective relationships with internal teams, external agencies, and patients.
- Strong organisational skills and attention to detail for record-keeping, training planning, and administrative responsibilities.
- Commitment to personalised care, reducing health inequalities, and empowering individuals to manage their health independently.
Essential
- Educated to degree level or relevant level of skills and experience
- Evidence of continuing education and development
- Experience within a healthcare setting
- Experience of working within a multidisciplinary team
Desirable
- Accredited Health and Wellbeing Coach (Personalised Care Institute (PCI) Accredited Courses)
- Experience of working within Primary Care
- Experience working within digital healthcare
Essential
- Working knowledge and a commitment to team approach and multi-disciplinary working
- Understanding of clinical risk management and clinical governance
- Knowledge of various IT systems and good IT proficiency
- An understanding, acceptance and adherence to the need for strict confidentiality
- Knowledge and awareness of issues affecting Primary Care and more broadly within the NHS
Desirable
- Knowledge of EMIS, SystmOne, Patches and Surgery Connect
