• Engage with individuals to conduct needs assessments and contribute to crafting care plans in line with the Care Act 2014.

• Coordinate and monitor care packages, liaising with multidisciplinary professionals such as GPs, nurses and social workers.
• Champion independence and person centred care, promoting well being for service users.
