GPwER in Frailty - Same-Day Care Clinician (Rural)

NCIC is expanding its Frailty services to integrate between community and secondary care in Carlisle. We are recruiting dynamic clinicians to join a multi-disciplinary team delivering a Same Day Care frailty pathway.

The role involves embedding standards, working across ED and primary care, and contributing to improved patient flow while maintaining high quality and safety in care for older people living with frailty.
