Job Description

£48,000 - £125,000 per annum Indemnity paid + BMA model + 8 weeks leave

An outstanding opportunity to join a dynamic GP Training practice which is excellently led and embracing the changes within Primary Care. Having recruited both Salaried GPs and a GP Partner for this practice, we can say with confidence you will be supported, looked after and given "artistic license" to contribute ideas and suggestions as to how things can be improved, changed and evolved.

With the implementation of Integrated Neighbourhood Teams, there is now the opportunity for a GP with a passion for and deep understanding of elderly patients to join the team, specifically to manage that patient cohort as either your sole position (8 10 sessions) or part of a portfolio career (4 6 sessions).

Salary

c£12,000 - £12,500 per session plus indemnity provided

Contract basis

12 or 24 months as preferred but with excellent scope for being made permanent

The surgery
  • Established GP Training Practice with numerous Trainers
  • Stable, experienced Partnership
  • Long standing team of loyal Salaried GPs
  • High dispensing
  • Geared towards reducing GP workload
    • Ongoing workflow projects to further decrease GP document workload
    • Visiting Paramedic or ANP doing home visits every day
    • GP support includes an ANP, three Paramedics, Mental Health Practitioner, First Contact Physio, Specialist Nurses (Diabetes, CDM and Women's Health) and two Pharmacists
    • Remote GP providing weekly admin support
  • Coffee break every morning
Your role
  • GPwER Frailty
  • Holding a list of elderly and frail patients only
    • 30 minute F2F appointments in practice
    • 1 hour - 1.5 hour home visits conducting comprehensive care evaluations
    • Care home oversight, rounds and reviews
  • Working collaboratively with wider team including Clinical Pharmacists, Pharmacy Technicians, Paramedics, Frailty Practitioners, Physiotherapists, Occupational Therapists and Chronic Disease Nurses
  • Participating in and often leading MDT meetings
  • Up to 10 sessions available with a minimum of 4 sessions required
