GP Clinician: Patient Care & Team Collaboration

Key Responsibilities Deliver high-quality, evidence-based clinical care to registered patients.

Assess, diagnose, and manage acute, chronic, and complex conditions.

Conduct a mixture of face-to-face and telephone consultations.

Work collaboratively with GPs, nurses, and other members of the multidisciplinary team.

Contribute to continuity of care, patient safety, and clinical governance.

Support quality improvement initiatives, enhanced services, and practice development projects.

Participate in practice and PCN meetings, audits, and service planning activities.
