NHS Jobs is seeking a dedicated and professional Clinical Pharmacist to join our team in Grantham. As a pivotal member of the multi-disciplinary team, you will work in a patient-facing role, managing chronic disease and conducting clinical medication reviews for patients with complex medication regimens. Your role will involve direct collaboration with the Pharmacist and Dispensary team, enhancing the quality of care provided to our patients.
Key Responsibilities- Chronic Disease Management: Manage patients with chronic conditions, ensuring appropriate and effective treatment.
- Clinical Medication Reviews: Conduct medication reviews to manage polypharmacy and optimize patient outcomes.
- Prescription Support: Assist general practice staff with prescription queries, support repeat prescriptions, and manage acute requests.
- Medicines Reconciliation: Oversee medication reconciliation during patient transfers, ensuring safe prescribing.
- Clinical Leadership: Provide medicines advice, drive medicines optimization, and contribute to quality initiatives.
- Community Integration: Facilitate collaboration with community and hospital pharmacies to improve healthcare access and outcomes.
- Quality and Outcomes Framework: Contribute to service improvements under the framework.
- Non-Medical Prescribing: Receive training to develop into a non-medical prescriber if needed.
- Registered pharmacist with professional registration.
- Experience in clinical settings, especially general practice.
- Knowledge of chronic disease management and polypharmacy.
- Excellent communication and teamwork skills.
- Leadership ability and commitment to quality improvement.
