SummaryNHS Jobs is seeking a dedicated and professional Clinical Pharmacist to join our team in Grantham. As a key member of the multi-disciplinary team, you will work in a patient-facing role, managing chronic disease and conducting clinical medication reviews for patients with complex medication regimens. Your role will involve collaboration with the Pharmacist and Dispensary team, enhancing the quality of care provided to our patients.
Key Responsibilities- Chronic Disease Management: Manage patients with chronic conditions, ensuring effective treatment.
- Clinical Medication Reviews: Conduct medication reviews to optimize patient outcomes in cases of polypharmacy.
- Prescription Support: Support general practice staff with prescription queries, manage the repeat prescription system, and handle acute requests.
- Medicines Reconciliation: Oversee medication reconciliation during patient care transfers to ensure safety and efficacy.
- Clinical Leadership: Provide medicines advice, promote medicines optimization, and contribute to quality initiatives.
- Community Integration: Facilitate collaboration with community and hospital pharmacies to improve service delivery.
- Quality and Outcomes Framework: Contribute to the management of quality and outcomes initiatives.
- Non-Medical Prescribing: Receive training and support to become a non-medical prescriber if desired.
- Registered pharmacist with professional registration.
- Experience in clinical settings, especially in general practice.
- Knowledge of chronic disease management and polypharmacy.
- Strong communication and teamwork skills.
- Ability to lead clinically and support quality improvements.
