The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 125 physicians across 25 sites within London, and Middlesex, Oxford and Elgin counties.
The Thames Valley Family Health Team (TVFHT) is seeking a dedicated and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our growing team to deliver foot care and wound care services. This position offers a unique opportunity to make a difference in the lives of those impacted by homelessness, mental illness, substance use, poverty, complex and/or chronic health conditions while being supported by a team of clinicians, Outreach Workers, and a body of community partners. You will also work in another London clinic providing primary care foot care services. You must be open minded to travel to different sites and be flexible to transition between different Electronic Medical Records and team cultures. Excellent organizational skills are imperative to the success of the candidate as is the physical capability to transfer equipment between sites when required.
SKILLS & QUALIFICATIONS REQUIRED:- Current registration and in good standing with the College of Nurses of Ontario
- National Competencies for Advanced Nursing Foot Care in Canada (CAFCN)
- Best Practice Guidelines in Wound Care (RNAO & provincial standards)
- Specialized training in providing comprehensive foot care is required.
- Experience in wound care assessment and treatment planning
- Culturally safe and trauma-informed approach to care
- Excellent interpersonal skills re: with patients and their families
- Must be team focused and flexible; works collaboratively with the multidisciplinary team.
- Ability to multi-task, work under pressure, and meet deadlines.
- Awareness and understanding of the social determinants of health.
- Commitment to the underlying principles of harm reduction
- Able to provide episodic care under a medical order or directive and work independently.
- Knowledge and experience working collaboratively with community resources, and an interest in systems navigation.
- Conduct holistic health assessments and develop patient-centered care plans
- Understanding of the Chronic Disease Management and Patient Self-Management Models
- Proficiency in the use of computer and with Electronic Medical Records (EMR)
- Must have access to reliable transportation as travel between sites is required
- This position is 100% on site.
