Job Description - Family Support Practitioner

Job title: Family Support Practitioner

Employer: Home-Start Telford & Wrekin, Meeting Point House, Southwater Square, Telford, TF3 4HS

Hours: 16 hours per week. Worked flexibly to meet the needs of the organisation.

Responsible to: Scheme Co-Managers

Purpose of the job:

Responsibilities

Main responsibilities:

Support for families:

  • Promoting, organising and facilitating groups for parents and their children. Accepting referrals passed on by colleagues and assessing the needs of families. Introducing families to appropriate support. Ensuring support for families is reviewed at regular intervals and at the end of Home-Start support, in line with current Home-Start guidance. To undertake the designated responsibilities for safeguarding and promoting children's welfare. Being responsible for recording statistical data regarding family activity and providing related information to the Family Support Coordinator / Strategic and Operational Managers

Supporting volunteers:

  • Matching and introducing volunteers to families. Providing support and supervision to volunteers. Being responsible for recording statistical data relating to volunteer activity and providing related information to the Scheme Co-Managers and wider team.

General:

  • Supporting with the implementation and review of Home-Start policies and procedures. Actively promoting the work of the scheme Implementing the organisation's administration and financial systems as relevant to the role, including use of the organisation's CRM system.

Managing the wider context:

  • Promoting the organisation, its profile, ethos and practice.

The post holder may be required to undertake any other duties that fall within the nature of the role and responsibilities of the post detailed above.
