Falls Response Community Support Worker - Integrated Referral Hub

Wye Valley NHS Trust is seeking a motivated Community Support Worker for the Integrated Referral Hub Falls Response. You will support adults at home, providing timely assessment, safe assistance, and timely referrals to prevent hospital admissions.

You'll also work within the Single Point of Access HUB, answering calls, handling referrals, and supporting wider teams. Roles include 12-hour shift patterns and some weekend work as part of rotational duties.
