We are looking for an Exercise Referral Professional to join our NERS team.

Key Responsibilities

• Provide consultations for clients and collect data to monitor progress


• Deliver a range of high-quality physical activity opportunities that are progressive and reflective of the needs of special populations in the local community


• Implement strict data and management protocols including GDPR compliance


• Assist in the evaluation of NERS by collecting accurate data


• Signpost clients to appropriate exit routes


• Support any new Exercise Referral Professionals and ensure they understand their role in the scheme


• Attend and represent the NERS Coordinator at quarterly meetings if required


• Ensure the development and dissemination of good practice in exercise delivery by researching evidence regarding exercise and medical conditions


• Provide cover for other Exercise Referral Professionals where necessary


• Deliver fitness classes and maintain a safe and welcoming environment at Knighton Sports Centre

Administrative Duties

• Telephone communications


• Distribution of appropriate mail


• Booking appointments


• Monitoring scheme attendance and client programmes


• Ensuring client follow-up procedures are implemented


• Collate information regarding the scheme and share it with the Coordinator


• Maintaining accurate and up-to-date records on the NERS Theseus system, the official patient and participant management database, including monthly monitoring and reporting in line with Public Health Wales requirements.

Qualifications

• Hyfforddwr campfa uwch lefel 3.


• Cymhwyster Atgyfeirio Meddyg Teulu lefel 3 cydnabyddedig.


• Cymhwyster lefel 4 cydnabyddedig neu'n gweithio tuag at un.


• Cymhwyster gwaith grŵp Lefel 2 (e.g. circuits/exercise to music).


• Cymhwyster Cymorth Cyntaf a CPR.


• O leiaf ddwy flynedd o brofiad mewn amgylchedd Iechyd a Ffitrwydd gan gynnwys cychwyn a darparu gwasanaethau Iechyd a Ffitrwydd i'r cyhoedd a phoblogaethau arbennig.


• Os yw'r ymgeisydd i weithio gyda chyflyrau lefel 4, o leiaf 2 flynedd o brofiad mewn cynllun atgyfeirio ymarfer corff.


• Hyddysg yn y defnydd o TG ac yn gymwys yn y defnydd o lwyfannau Microsoft Office a chyfryngau cymdeithasol, yn enwedig y gallu sy'n gysylltiedig â phob agwedd ar gyflwyno sesiynau rhithwir, cynnal a mynychu cyfarfodydd rhithwir, cyflwyno asesiadau / ymgynghoriadau wyneb yn wyneb a rhithwir, a chwblhau'r holl gyfrifoldebau swyddfa / gweithio gartref.


• Profiad o ddarparu cyfarwyddyd ffitrwydd a dylunio rhaglenni i gleientiaid.


• Cydlynu, datblygu a chynllunio gweithgareddau hamdden mewn ystod eang o amgylcheddau gan gynnwys lleoliadau cymunedol/addysgol.


• Hyder a chymhwysedd wrth gyflwyno rhaglenni/dosbarthiadau ymarfer corff ar gyfer poblogaethau arbennig.


• Bod yn ymroddedig i ofal cwsmeriaid ac i ddarparu gwasanaeth o ansawdd effeithlon ac effeithiol.


• Meddu ar sgiliau cyfathrebu rhyngbersonol, ysgrifenedig ac ar lafar da gyda'r gallu i siarad â chleientiaid / cwsmeriaid wyneb yn wyneb, ar y ffôn ac yn ysgrifenedig.


• Ymwybyddiaeth o GDPR.


• Sgiliau trefnu da i reoli llwyth gwaith yn effeithiol a sicrhau casglu data cywir.


• Meddu ar y gallu i wneud penderfyniadau a thawelu sefyllfaoedd anodd.


• Gallu gweithio gyda menter eich hun a gweithio mewn amgylchedd tîm trwy ddatblygu perthnasoedd gwaith effeithiol ac adeiladol gyda chydweithwyr.


• Yn gallu teithio'n annibynnol.


• Ymrwymiad i ymgymryd â hyfforddiant pellach.

• Health and Fitness related Degree.


• Registered at Level 3/4 CIMSPA.


• Walking Way to Health leaders training.


• Defibrillator training.


• Experience of working on a GP Exercise Referral Scheme or a Chronic condition rehabilitation scheme.


• Welsh Language skills are desirable.

Hours: 14 hours per week (12 awr fel Gweithiwr Proffesiynol Atgyfeirio Ymarfer NERS, 2 awr fel Hyfforddwr Ffitrwydd)
