Evening Practice Nurse: Chronic Care (Flexible Sessions)

EBPC is seeking a Practice Nurse to join our extended service in Bracknell. You will deliver high-quality nursing services to the entitled patient population and work with the multidisciplinary team to ensure comprehensive care.

Responsibilities include health promotion, chronic disease management, and women/men's health services within your scope. You will support policy review and maintain high standards of care in a collaborative environment.
