Job Description
End of Life Care Facilitator

The closing date is 01 October 2025

An exciting opportunity has arisen for a part-time (15 hrs/week), permanent End of Life Care Facilitator post due to partial retirement of the current postholder. The postholder will be based in the Palliative Care Service at the Homerton Hospital site, which provides specialist palliative and end-of-life care to adult patients and people important to them at the hospital and Mary Seacole Nursing Home.

The postholder will combine clinical, educational, programme development, audit, and quality improvement skills to promote quality end-of-life care across the Trust. They will facilitate and support the dissemination, implementation and evaluation of the Palliative and End of Life Care Strategy within the Homerton Healthcare NHS Foundation Trust and engage with key stakeholders within the Trust.

Main duties of the job
  • Provide expert nursing support in end-of-life care (EoLC), working alongside and supporting generalist staff.
  • Act as a clinical role model, offering education, advice, and emotional support to patients, families, and staff.
  • Assist teams in recognising dying, managing symptoms, and avoiding inappropriate interventions.
  • Facilitate advance care planning and support patient and family involvement in decision-making.
  • Lead and influence improvements in palliative and EoLC across the Trust.
  • Develop, deliver, and evaluate education and training programmes for non-specialist staff.
  • Mentor students and early-career clinicians in providing quality EoLC.
  • Collect and analyse data to monitor and improve service delivery; contribute to audit, quality improvement, and research.
  • Support implementation of Trust-wide EoLC strategy, fostering collaboration, shared governance, and best practice.
  • Maintain professional standards and continuous professional development in line with NMC requirements.

  • NMC Registration
  • Bachelor's degree in nursing or equivalent
  • Evidence of professional development
  • Recognised teaching qualification or working towards
  • Care of the dying person and family course/equivalent
  • Advanced communication course or prepared to work towards
  • Postgraduate qualification in palliative care or relevant discipline
Skills/ Abilities

Experience
  • Experience of and interest in end-of-life care
  • Relevant clinical experience demonstrating transferable skills to the palliative care setting

  • Experience of managing a team
