Job Description
Overview

Rotherham Enabling Service works with people in the community to help them learn new skills or increase confidence in their daily activities. Services are aimed at people who may have experienced a spell of ill health or a change in circumstances and need short term support to return to previous levels of independence. Directorate: Adult Care, Housing & Public Health.

Key Responsibilities
  • Visit people in their own homes and monitor how direct support is delivered.
  • Set up services, complete risk assessments and care plans, review care and support needs, and monitor goals and achievements.
  • Adjust support calls and end services in conjunction with people being supported.
  • Liaise with Coordinators to ensure a streamlined service and smooth transition to greater independence.
  • Attend multidisciplinary team meetings and support referrals to health professionals.
  • Support and monitor Enabling support workers, undertaking observed practice in the community.
Working Conditions

Location: Rawmarsh Joint Service Centre, Rotherham. Full time, 37 hours per week, permanent. The service operates from 6:45 am until 10 pm daily, including bank holidays and weekends (enhancement paid). Travel across the Rotherham borough is essential; using a car is not required but the postholder must be able to travel within a reasonable timescale. This post does not meet Home Office criteria for a certificate of sponsorship visa category.

Qualifications & Clearances

This post requires an enhanced level Disclosure and Barring Service check (criminal records check). Confirmation is also subject to a medical assessment.
